hth 5 way test kit ?

i lost my directions for this kit this winter (building a new house)
does anyone know the directions for cya test ?

mix equal parts pool water and reagent in the vial, cap and shake for 30 seconds, let sit 2 minutes and shake again, pour into tube slowl until black dot disappears.
